Simplifying Free Nipple Graft Mammoplasty in Postmenopausal Obese Patients using Inferior Dermaglandular Flap

Breast hypertrophy causes medical problems ranging from back and neck pain to dermatitis creating a demand for breast reduction surgery along with aesthetic psychological reasons. Free-nipple-graft is safeguard procedure patients who are at risk nipple necrosis pedicled techniques but has disadvantage of resultant flat breasts poor projection. Study included twenty-three postmenopausal women underwent inferior dermaglandular flap free-nipple-grafting, between September 2015 January 2020. A rectangular 8 × 10 cm inferior-pedicled was dissected anchored the thoracic wall better projection upper pole fullness. All had least one comorbidity. Visual analog scale evaluated 1–3 months. Mean age 54.08 SD 4.65 years, mean body mass index (BMI) 33.56 2.53. follow-up period 10.56 5.6 SN-N distance 37.45 3.77 38.86 3.91 right left breasts, respectively. resection weight 1373 440 g. Wound dehiscence seen in 3 (13.0%) treated medically. No other complications, including total areolar complex (NAC) loss, were seen. visual score patient satisfaction 7.7. satisfied their final shapes. Utilizing technique free-nipple-graft mammaplasty provides more aesthetically pleasing shape contour. It helps achieve an effortless preoperative planning straightforward high obese women.
